ACCEPTANCE OF TERMS
By accessing our website, scheduling an appointment, or receiving medical services from Olympus Health (“we,” “us,” or “our”), you (“you,” “your,” or “patient”) agree to be bound by these Terms of Service. These terms constitute a legally binding agreement between you and Olympus Health.
If you do not agree to these terms, please do not use our services or website.
MEDICAL SERVICES OVERVIEW
Direct Primary Care Services
Olympus Health provides direct primary care services to patients in Texas, including:
- Primary medical care and consultations
- Chronic disease management
- Hormone replacement therapy
- Obesity medicine and weight management
- Preventive care and health optimization
- Prescription management and monitoring
Scope of Practice
Our physicians are licensed in Texas and provide care within their scope of practice. We do not provide:
- Emergency medical services
- Surgical procedures
- Specialized procedures requiring facility-based care
- Services outside of primary care medicine
ELIGIBILITY AND REQUIREMENTS
Patient Eligibility
To receive our services, you must:
- Be 18 years of age or older
- Be a resident of Texas
- Provide accurate and complete health information
- Agree to participate actively in your care plan
Membership Requirements
- Complete and accurate enrollment information
- Payment of monthly membership fees
- Compliance with treatment plans and recommendations
- Respectful communication with staff and providers
- Adherence to appointment scheduling policies
PAYMENT TERMS AND BILLING
Membership Fees
- Direct Primary Care: $150 per month
- Return to Health: Custom pricing based on services
- Fees are due monthly in advance
- No refunds for partial months of service
Additional Costs
The following are NOT included in membership fees:
- Medications: Prescribed by us, filled and paid for by you
- Laboratory Tests: Ordered by us, paid for by you
- Imaging Studies: X-rays, MRIs, etc. when needed
- Specialist Referrals: When specialized care is required
- Emergency Care: Hospital and emergency services
Payment Methods
- Automatic monthly billing to credit/debit card
- ACH bank transfers
- Payment must be current to maintain active membership
- Failed payments may result in service suspension
Late Payments and Collections
- Services may be suspended for non-payment
- Late fees may apply after 10 days
- Unpaid balances may be sent to collections
- You are responsible for collection costs and legal fees
APPOINTMENT AND COMMUNICATION POLICIES
Scheduling
- Appointments available during posted business hours
- 24-hour notice required for cancellations
- No-shows may result in fees or service restrictions
- Emergency appointments subject to availability
Communication Access
- Direct communication with physicians via text, phone, email
- Response within 24-48 hours for non-urgent matters
- Same-day response for urgent (non-emergency) issues
- Emergency situations require calling 911
Telemedicine Services
- Most consultations conducted via secure video platform
- Stable internet connection and compatible device required
- You are responsible for ensuring privacy during calls
- Some conditions may require in-person evaluation
PATIENT RESPONSIBILITIES
Accurate Information
You agree to:
- Provide complete and accurate medical history
- Disclose all current medications and supplements
- Report changes in health status promptly
- Update contact and insurance information as needed
Treatment Compliance
You agree to:
- Follow prescribed treatment plans
- Take medications as directed
- Complete recommended laboratory tests
- Attend scheduled appointments
- Communicate concerns or problems promptly
Respectful Behavior
You agree to:
- Treat all staff and providers with respect
- Communicate professionally and appropriately
- Avoid abusive, threatening, or discriminatory language
- Respect office policies and procedures
Emergency Situations
You understand that:
- Olympus Health does not provide emergency services
- You must call 911 for medical emergencies
- You should go to emergency room for urgent situations
- We are not responsible for emergency care decisions
PHYSICIAN-PATIENT RELATIONSHIP
Establishment of Relationship
The physician-patient relationship is established when:
- You enroll as a patient
- A physician accepts you for care
- Medical services begin to be provided
- This relationship continues during active membership
Scope of Relationship
Our physicians will:
- Provide care within their scope of practice
- Exercise independent medical judgment
- Maintain appropriate professional boundaries
- Protect your confidential health information
Termination of Relationship
The relationship may be terminated by:
- You: By canceling membership with 30 days notice
- Us: For non-payment, non-compliance, or inappropriate behavior
- Mutual Agreement: When care is no longer needed
- Physician Retirement/Departure: With appropriate notice and transition
WEBSITE TERMS OF USE
Permitted Use
You may use our website to:
- Learn about our services and physicians
- Schedule appointments and communicate with us
- Access patient portal and health information
- Complete forms and provide required information
Prohibited Use
You may not:
- Use the website for illegal or unauthorized purposes

INDEMNIFICATION
You agree to indemnify and hold harmless Olympus Health, its physicians, employees, and affiliates from any claims, losses, damages, or expenses arising from:
- Your violation of these terms
- Your misuse of our services or website
- Your violation of any third-party rights
- Any false or misleading information you provide
FORCE MAJEURE
We are not responsible for delays or failures in performance due to circumstances beyond our reasonable control, including:
- Natural disasters or severe weather
- Government actions or regulations
- Internet or technology failures
- Public health emergencies
- Other unforeseeable events
DISPUTE RESOLUTION
Governing Law
These terms are governed by the laws of the State of Texas, without regard to conflict of law principles.
Jurisdiction
Any legal disputes will be resolved in the state or federal courts located in Denton County, Texas.
Mediation
Before filing any lawsuit, both parties agree to attempt resolution through mediation with a mutually agreed-upon mediator.
Class Action Waiver
You agree that any disputes will be resolved individually and waive the right to participate in class action lawsuits.
MODIFICATIONS TO TERMS
Right to Modify
We reserve the right to modify these Terms of Service at any time. Changes may be made to reflect:

TERMINATION

You may terminate your membership:

We may terminate your membership immediately for:
- Non-payment of fees
- Violation of these terms
- Inappropriate or abusive behavior
- Providing false information
- Any illegal activity
Effects of Termination
Upon termination:
- Access to services will cease
- You remain responsible for unpaid fees
- Confidentiality obligations continue
- Medical records will be maintained per legal requirements
SEVERABILITY
If any provision of these terms is found to be unenforceable or invalid, the remaining provisions will continue in full force and effect. The invalid provision will be replaced with a valid provision that most closely achieves the original intent.
ENTIRE AGREEMENT
These Terms of Service, together with our Privacy Policy and any signed enrollment agreements, constitute the entire agreement between you and Olympus Health regarding our services.
